Financial Giving Request Forms Available
Financial giving request forms are available for those who would like to receive a summary of your 2018 financial giving to the church. Request forms must be submitted in order to receive your report. Reports will not be processed without a submitted form.
Important information about Online Giving statements
If you have given through our online giving platform this year, you will receive a separate statement for just your online giving on January 20, 2019 via email.
The SRCC form is only for any contributions made through the offering envelopes in person. If you have not made any financial contributions in person this year, you do not need to fill out the request form.
To Submit
“Offering Total for Tax Return” paper forms are being provided on Sundays along with the bulletin. To submit in person, please submit it in the offering basket on Sundays or at the SRCC Main Office (located at 1111 N. Brookhurst St. Anaheim, CA 92801).

When will I receive my report?
Reports will be available and sent out via email or mail (depending on your selection in the form) starting Sunday, January 20, 2019.
Deadline: Sunday, December 23
